How do I assign a questionnaire?

Questionnaires can be assigned to patients in neurocare OS to collect patient-reported information and track outcomes over time.

 Assigning a questionnaire

  1. Open the patient's record.
  2. Navigate to the Questionnaires section.
  3. Select Assign Questionnaire.
  4. Choose the questionnaire you would like to assign.
  5. Configure any available settings, such as repetitions or scheduling.
  6. Save the assignment.

Once assigned, the questionnaire will be available for the patient to complete.

Reviewing questionnaire assignments

Assigned questionnaires can be viewed within the patient's record, where you can monitor their status and review completed responses.

Troubleshooting

I cannot assign a questionnaire

Verify that:

  • The patient record has been created.
  • You have permission to manage questionnaires.
  • A questionnaire is available for assignment.
